Pielonefritis xantogranulotamosa: reporte de 15 casos y revisión de la literatura
Abstract
Fifteen cases of Xantogranulomatous pyelonepbritis diagnosed at Hospital San Juan de Dios in Bogota, are presented. The disease ocurred in middle aged women, particularly in those with history of urinary infection (33%). The most common complaints were abdominal pain, fever and lumbalgia, while fever, pallor, palpable mass and lumbar pain evoked by percussion were the most frecuent signs disclosed by medical examination. Proteus mirabilis (55%) and Escherchia coli (22%), were the germs more commonly isolated from patients' urine specimens. Lithiasis and renal exclussion were the most remarkable roengtenologic findings, whereas lithiasis, increased renal size and hydronephrosis vs. pyonephrosis pattern were seen in echografic examination. Gammagraphic examination disclosed functional exclussion of the kidney and obstructive pattern and the CAT scan allow a definite diagnosis and stablished the extension of the disease. Just 33% of the patients arrived with diagnosis of Xantogranulomatous pyelonephritis to the operation room. A brief review of the literature is presented.
