Infant With Adrenal Insufficiency and Shock

Abstract Introduction Adrenal insufficiency is an uncommon presentation to the emergency department (ED) but can be the cause of morbidity and mortality if not recognized and treated appropriately. In this module, we include an actual case of an infant who presented to a pediatric ED. Methods The purpose of this module is to teach health care professionals how to recognize the signs and symptoms of adrenal insufficiency in a neonate presenting in shock. In addition, learners review the basic stabilization using pediatric advanced life support algorithms and medical management of a patient with uncompensated shock from adrenal insufficiency. Results This module has been used with pediatric residents and pediatric emergency medicine fellows. Approximately, 10 learners have completed this module during two separate sessions. After the session, we have had good feedback about the scenario. Learners gave feedback that this is a good case for review of adrenal insufficiency and fluid resistant hypotension. They all felt more comfortable recognizing and treating a patient with adrenal insufficiency. Discussion Adrenal insufficiency is an uncommon presentation to the emergency department but can be the cause of morbidity and mortality if not recognized and treated appropriately. This module teaches health care professionals how to recognize the signs and symptoms of adrenal insufficiency in a neonate presenting in shock.
