Long-term indwelling trans-anal ileus tube placement and nursing management for an elderly patient with intestinal obstruction (1例高龄肠梗阻患者长期留置经肛型肠梗阻导管的护理体会)

Abstract

This article summarized the nursing practice of an elderly patient with acute exacerbation of chronic cholecystitis complicated by intestinal obstruction during long-term indwelling trans-anal ileus tube placement. The key points of care are to improve the fixing method to prevent the tube dislocation; Protect the skin and prevent the occurrence of pressure ulcers; Keep the tube unobstructed to ensure effective drainage; Develop a diet plan to meet nutritional intake; Exercise reasonably to prevent complications; Strengthen psychological care and promote the recovery of the patient. The abdominal pain and bloating symptoms had been relieved after treatment and nursing by multi-disciplinary team, and the trans-anal ileus tube had been successfully removed. (本文总结1例慢性胆囊炎急性发作合并肠梗阻的老年患者长期留置经肛型肠梗阻导管的护理经验。护理重点包括合理固定, 防止管路脱出; 保护皮肤, 预防压疮发生; 保持管路通畅, 确保有效引流; 制定饮食方案, 满足营养摄入; 合理锻炼, 预防并发症; 加强心理护理, 促进患者康复。经过多学科联合诊治和护理, 患者腹痛、腹胀明显缓解, 肠功能较前恢复, 最终成功拔除经肛型肠梗阻导管。)
