A comparative clinical study of ayurvedic formulations (Phalatrikadi Yoga Ghana Vati and Lekhaniya Mahakashaya Kwatha) and metformin in the cases of Medoroga with special reference to metabolic syndrome

Introduction: Obesity and metabolic syndrome (MS) are interconnected disorders having worldwide prevalence, which are associated with a lot of abnormalities such as physiological, biochemical, clinical, and metabolic. Both disorders affect the risk of cardiovascular disease and diabetes. It has a strong association with premature morbidity, mortality, and reduced quality of life (QoL) in the wake of rapid industrialization, overmuch energy supplementation, and unhealthy life routine. Weight management and a wholesome regimen are major and fruitful methods for the deterrent and treatment of MS. Phalatrikadi Yoga Ghana Vati with Lekhaniya Mahakashaya Kwatha as Anupana was selected as a trial drug and compared its safety and efficacy with parallel control drug metformin in the obese patients associated with the MS. (Metformin is a very potent allopathic drug. The Ayurvedic herbal drug is soft in potency. Single Ayurvedic drug is not much effective as compared to metformin in previous research studies. That's why, Ayurvedic combinations were selected). Methods: The safety and efficacy of Phalatrikadi Yoga and Lekhaniya Mahakashaya Kwatha were evaluated against MS. Ninety volunteers were enrolled and allocated into two groups. Patients were treated with Phalatrikadi Yoga and Lekhaniya Mahakashaya Kwatha in the 1st Group (A). Patients were treated with metformin in the 2nd Group (B). The time duration was 3 months, including three follow-ups for this research work. Results: Eighty-two patients have completed the course of treatment. Ayurvedic formulations have shown a significant correction in subjective parameters, lipid profile, body mass index, body weight, and Agni status score. There was also marked improvement in blood pressure, waist circumference, waist-to-hip ratio, blood sugar level, erythrocyte sedimentation rate, and QoL score with hepatic, renal, and cardiac protection. Conclusion: Phalatrikadi Yoga and Lekhaniya Mahakashaya Kwatha were found safe and more effective than metformin in the management of MS.
