The occurrence of stroke in patients undergoing primary percutaneous coronary intervention (PCI) is infrequent but the associated events could be catastrophic. The in-hospital mortality and morbidity rate of such patients is much higher than other procedure related complications. In spite of taking all precautions, such dreaded events may rarely occur. We report one such dreaded event that happened in one of our patients after primary PCI which was managed conservatively.
