This article identifies the critical links that characterize the response to the public health emergency of SARS-CoV-2 in a case study approach in the peripheral insular specificity of the Dominican Republic. The adversities and dependency are analyzed and characterized in the context of international determinations and health geopolitics in the face of a global pandemic, a second critical link, the epidemiological characterization of the 2020 period of the epidemic at the country level and in comparison with the region, on the other hand , studying decision making and theoretical frameworks operationalized by Dominican epidemiology and public health governed by a clinical biomedical epistemology in the design and management of institutionally condensed interventions in experimental trials of quarantines, neohygienism, punitivism and health vigilantism; thirdly, articulated to a new preeminent grammar of the national sociopolitical language through the presence of the government of the "experts" with the institutionalization of public-private partnerships, and the fourth link, the description of the institutional arrangements in a health systems fragile, segmented, financialized from the neoliberal reform of 2001. Finally, on the basis of latin american critical thinking in health, possible complementary lines of study are outlined, and the challenges of training and political-health action in the Dominican Republic from a geopolitics of the Greater Caribbean.
