Many intracranial variations of middle cerebral artery, which occur infrequently compared to other intracranial vascular anomalies, are described in the literature. We describe a case of incidentally detected accessory middle cerebral artery on digital subtraction angiography. An understanding of such variations is important for those performing intracranial aneurysm surgeries. These variations may also serve as a source of collateral supply in cases of ischaemia.
