Complex pathogenetic treatment of liver cirrhosis

Treatment of patients with liver cirrhosis (LC) and its complications is one of the most difficult problems of surgery, in particular, surgical hepatology and biliary surgery. According to WHO data, the frequency of LC is steadily increasing. According to these data the authors conclude that there is a lack of understanding of pathogenetic mechanisms liver parenchymatic cells cirrhotic lesion. Chronic experimental trials were carried out to reproduce the model of LC in rats by four-carbon monoxide peroral administration. In the rat blood 1, 6, 12 hours, 1, 3, 5 and 7 days after the experimental LC formation, proteolysis standard activity was determined through lysosomal enzymes activity measurement. It was found that LC manifestation is accompanied by activation in blood serum of rats of acid, trypsin-like proteases and lysosomal enzymes, indicating the generalization of the process. Catepsin D activation indicates the presence of inflammatory-destructive changes in the liver which reflects systemic inflammation in case of LC. The data obtained suggest that rats blood proteolysis activation is probably one of the components of pathogenesis of the disease, Both alpha-lipoic acid and tivortine positive effects are the experimental background of their clinical testing reasonability in patients with LC, both individually and in combination with hepatoprotective compounds.
