Congenital testicular exstrophy or scrotoschisis is an extremely rare congenital anomaly of the scrotal wall leading to testicular extrusion. The exact etiology and prevalence remain unknown. Mostly, it occurs unilaterally in the higher anterior aspect of scrotum. We report a case of bilateral scrotoschisis in an 8 h old newborn presenting in emergency department with a 4 × 2 cm skin and dartos defect present across the midline in the lower anterior aspect of scrotum causing exteriorization of both testes. Surgery was done to reposition the testes. Postoperative follow-up at two weeks showed healed scar.
