Extracellular matrix and left atrial remodeling in patients with ischemic chronic heart failure and renal dysfunction
Abstract
Remodeling occurs in the structural components of the myocardium – extracellular matrix, which is constantly synthesized, degraded and is an integral part of most pathological conditions of the heart. Aim. To identify the characteristics of remodeling extracellular matrix and left atrial in patients with ischemic chronic heart failure and renal dysfunction. Methods and results. Collagen metabolism markers (MMP-9, TIMP-1, РІСР), cystatin C activity were studied in 336 patients with ischemic chronic heart failure using ELISA method. Structural and functional parameters of the heart were detected by echocardiography. It was established that the moderate renal dysfunction presence in ischemic chronic heart failure is associated with age, history of atrial fibrillation, accompanied by the increasing of NT-proBNP, cystatin C, MMP-9, reduced TIMP-1 and is characterized by structural remodeling of the left atrium with increased systolic pressure in the pulmonary artery, the deterioration of systolic and diastolic function. Conclusion. Left atrium pathologic remodeling in patient with CHF is important part of the structural and functional changes of the heart, that is accompanied with extracellular matrix balance disturbances and renal functioning decrease.
