Patent omphalomesenteric duct in a newborn; a rare finding and presentation

Abstract

The omphalomesenteric duct is an embryologic structure connecting the yolk sac with the developing fetus. Normally it involutes but sometimes the duct persists, which is most commonly found as a Meckel's diverticulum. Meckel's diverticulum typically presents in pediatric patients under 10 years of age as rectal bleeding or melena. Other complications range from obstruction, perforation, or inflammation. Here we present a neonate that initially presented with discharge from his umbilicus that progressed to drainage of stool. Imaging and workup prior was unremarkable. Surgical resection revealed the presence of a Meckel's enterocutaneous fistula to the umbilicus.
