Background: Psychomotor physiotherapists use their own experiences and sensations as tools in their clinical work, which can make them prone to emotional exhaustion and burnout. To be able to safeguard the relationship with the patient, promote the patient’s development and at the same time safeguard themselves as therapist, a greater understanding is warranted as to how psychomotor physiotherapists can handle challenges in the relationship with the patient. Objective: The objective of this study is to shed light on psychomotor physiotherapists experiences with challenges in their relationship with patients. Method: This article is based on a qualitative study and has a phenomenological scientific framework of understanding. The data material is based on semi-structured in-depth interviews with four psychomotor physiotherapists. A thematic analysis of the material was used. Findings: The physiotherapists find the relationships with patients challenging when patients present with high expectations and does not assume responsibility for and commitment to the treatment process. In the experience of the physiotherapists, it is a matter of daring to face these challenges in a clear and, at the same time, a compassionate way. The challenges are described as opportunities for growth and change for the patients. Conclusion: This study contributes with an understanding of challenges within the patient relationship as a natural and necessary part of growth and change. Change and development on the part of the patients demand a willingness to take risks and work in the borderland between the pleasant and unpleasant, between self and the other. Qualities of the physiotherapists such as attention, acknowledgment and emotional presence, appear to be essential in this process of finding a balance.
