A pilot program for patient involvement in interdialytic weight gain management in dialysis patients

Higher interdialytic weight gain (IDWG) is associated with higher predialysis blood pressure, worse nutritional status and increased mortality. The aim of pilot program is to involve patients in the management of their IDWG in order to obtain better outcomes. Our pilot program will be based in two simultaneous approaches: Firstly, patients will be given a chart to fill out at the beginning of each dialysis session to register whether they have managed to keep their IDGW below their personalized target. Secondly, professional dietary counseling will be reinforced for those who present lower IDWG compliance on a monthly basis. Outcomes will be measured according to IDWG control, predialytic blood pressure control and nutritional status at 3 months. These parameters will be compared with to data form the previous trimester. All patients currently undergoing hemodialysis at our clinic (n=50) will be included. This program will be carried out by our hemodialysis clinical team including physicians, nurses and nutritionists. Our integrative approach aims for the delivery of person-centered, high-quality care, by empowering our patients to attain better health outcomes with their direct participation.
