FORMATION OF OBTURATOR APPARATUS IN LOWER UTERINE SEGMENT IN PATIENTS WITH INVASIVE CERVICAL CANCER AFTER RADICAL TRANSABDOMINAL TRACHELECTOMY
Abstract
A need to strengthen the lower uterine segment and the formation of «obturator» apparatus for carrying a subsequent pregnancy is one of the problems of transabdominal radical trachelectomy performed for patients with stage IA2–IB2 cervical cancer. Isthmo-cervical incompetence may lead to mechanical falling and prolapse of the fetal bladder, creating conditions for its infection. We have developed and tested a method for forming the obturator apparatus of the uterus at the level of the internal os. Usefulness of the mesh implant made of a superelastic titanium nickelid is due to its unique biomechanical properties and high biocompatibility. Regarding the problem of strengthening the utero-vaginal anastomosis, the implant takes over the basic function of the «obturator» apparatus of the uterus, providing functional support, and formed anastomosis to change intrauterine and intraperitoneal pressure.
