This article shows the results of a significant experience based on the implementation of the Todos a Aprender program in the Institucion Educativa Playas del Viento in the municipality of San Bernardo del Viento, in the department of Córdoba. The Learning Communities and the Formative Assessment are addressed as tools for the identification of difficulties, strengths and for the improvement of the teaching practice. The experience that has been carried out in the institution has a special element, and it is the use of the results of the students' learning in the external tests. The analysis of these results was key to propose classroom didactic actions for the improvement of the difficulties and the planning of activities using tools provided by the program Todos a Aprender. This allowed the use of continuous assessment strategies, focused on the processes and oriented to follow up the learning difficulties of the students. Based on the results of the study and the experience, it can be considered that the consolidation of a learning community, the applied didactic actions and the formative evaluation practices contribute in a substantial way in the students' learning.
