This review provides information on the evolution of ophthalmic and obstetric approaches to management of pregnancy, childbirth in women with high myopia. It shows the problem of prevention in the threat of development of rhegmatogenous retinal detachment in childbirth in conjunction with the development of the diagnostic capabilities of retinal disease. It gives an assessment to a role of peripheral vitreochorioretinal dystrophies (PVCRD), as a leading pathogenetic risk factor for retinal detachment in childbirth. The principal modern strategies of active management of pregnant women with PVCRD during pregnancy and childbirth are given. Indications for vaginal birth, or to the exclusion of vaginal birth if PVCRD are specified; there is given a modern assessment for the importance of the availability of high myopia in pregnant women.
