Background: Semen quality is an important index for evaluating male fertility and reflecting masculine reproductive health. Infertility caused by oligoasthenozoospermia (OAS) has become a significant problem that affects the overall quality of life. The development and application of safe and effective drugs for symptomatic treatment of OAS are of great significance for alleviating population decline and aging. Methods: A literature search was performed in the past 20 years in the PubMed, China National Knowledge Infrastructure (CNKI), and Wanfang Data Knowledge Platform (Wanfang) on the treatment of OAS by TCM, expands the dialectical thinking of TCM for OAS, and systematically assesses the pharmacological mechanism of Chinese herbal medicine (CHM) and its compound preparations in the therapy of this disease. Results: The theory of dialectical treatment of TCM has important guiding significance for the cure of OAS. CHM can be multi-targeted to improve the quality and quantity of sperm, regulating the overall symptoms of patients to a comprehensive treatment of oligospermia. Conclusion: This study can provide therapeutic ideas and theoretical basis for the treatment of OAS.
