COMPARATIVE ANALYSIS OF CLINICAL EFFICACY OSTEOPLASTIC MATERIALS FOR INCREASE THE ALVEOLAR BONE DURING SINUS – LIFTING OPERATION

Every year, more than 2 million surgeries are performed in the world, during which there is a need to replace lost bone tissue. Such interventions are common in surgery, traumatology, oncology and in particular in dentistry. The problem of bone regeneration is one of the most pressing in modern surgical dentistry and maxillofacial surgery. Dental alveolar defects of the jaw bones are very common and are a serious problem in dental treatment and rehabilitation. Defects of the bone tissue of the maxillofacial area can occur as a result of various pathological processes, namely: traumatic tooth extraction, aggressive periodontitis, osteomyelitis of the jaws, tumors (benign, malignant and tumor-like), odontogenic and neodontogenic cysts, congenital and congenital. All these factors create extremely unfavorable conditions for the treatment of both primary and secondary adentia, especially if the method of treatment is dental implantation. Thus, the development of new implant materials for dentistry is a modern and not completely solved problem. The article presents a comparative analysis of the clinical effectiveness of osteoplastic materials, in particular compositions based on multipotent mesenchymal stromal cells of adipose tissue and platelet-enriched plasma, in filling bone defects during sinus lifting. Criteria for the effectiveness of treatment at the stage of early postoperative period in the subjects was to assess the severity of pain, collateral edema and the degree of hyperemia of the oral mucosa after surgery. The aim of the study. To determine the clinical effectiveness of osteoplastic materials to increase the volume of the alveolar bone of the jaws during operation of sinus lifting. Object and methods of research. The study was conducted on the basis of Bukovynian State Medical University, Chernivtsi, Ukraine. Sinus lifting operation was performed in 67 patients. At the same time, in 30 people the operation was performed using osteoplastic material “Kolapan – L” (subgroup 1A); in 37 people, surgery was accompanied by the use of osteoplastic combination of multipotent mesenchymal stromal cells of adipose tissue (MMSC – AT) + “Colapan – L” + platelet-enriched plasma PRP (subgroup 1B). Statistical processing of the obtained results was performed on a personal computer using the licensed programs “Microsoft Excel” and “Statistica”. Research results and their discussion. It was found that in patients of subgroups B, where “Kolapan – L” + MMSCAT + PRP was used, the absence of pain at the final stage of postoperative observation was noted by 89.31% of persons, which was 1,2 times p1 0,05. In the final postoperative stages, collateral edema was absent in 98.47% of operated subgroups B, which exceeded the number of persons in subgroup A, where the bone defect was augmented by “Colapan – L” 1,2 times, p1 <0.05. The absence of hyperemia of SOPR was determined in the final postoperative follow-up period in 92,37% of patients of subgroups B, which was 1,3 times more than the values in subgroup A p1 <0.05. Conclusions. Comparative analysis of the clinical effectiveness of osteoplastic materials to increase the volume of the alveolar bone of the jaws during sinus – lifting operation convincingly proved the advantage of using the combination “Kolapan – L” + MMSС – AT + PRP, which was confirmed by subjective and objective symptoms postoperative period.
