Emergency pulmonary valve replacement in severe valvular pulmonary stenosis with refractory ventricular tachycardia- A life saving procedure

A 53 year old male presented to emergency ward with symptoms of breathlessness, palpitation and frequent episodes of loss of consciousness. An urgent 12 lead electrocardiogram revealed a non sustained ventricular tachycardia refractory to medication necessitating multiple DC cardioversion for recurrent ventricular tachycardia. Echocardiogram revealed severe pulmonary valvular stenosis with right ventricular dysfunction. He had a history of balloon pulmonary valvotomy twelve years back and surgical closure of atrial septal defect thirty five years back. Emergency pulmonary valve replacement was done with 25mm epic supra aortic bioprosthetic heart valve which resulted in resumption of normal sinus rhythm without any no further episodes of ventricular tachycardia during perioperative as well as in follow up.
