In recent years, the NKF National Kidney Foundation modified the term "diabetic nephropathy" which is only used for biopsy-confirmed pathology using the termof diabetic kidney disease (CKD) in order to understand that some patients with diabetes can have renal disease of another etiology, so it should be promptly referred to the nephrologist to diagnose and carry out the appropriate treatment. However, as we will develop in this exchange, currently the paradigm in the choice of medication for diabetes has been modified from the glucocentric to a metabolic cardiorenal perspective that has allowed us to combine some concepts to establish better choices in treatment and avoid the appearance and/or CKD progression. The patients we approach not only have diabetes but also have obesity beyond the high prevalence of hypertension, dyslipidemia and other glomerulopathies, etc. that can be submitted together.
