LIPOSARCOMA RECIDIVANTE DE PARTES BLANDAS: PRESENTACION DE UN CASO Soft Tissues recidivant lyposarcoma: Presentation of case
Abstract
El Liposarcoma es un tumor que hemos visto con poca frecuencia en nuestra práctica médica. En el presente trabajo, realizado en el Hospital Docente Clínico Quirúrgico Joaquín Albarrán Domínguez, en Ciudad de La Habana, 2004, le presentamos un caso que veníamos tratando hace más de una década. Se trata de una paciente de 79 años de edad con una neoplasia de esta variedad que ha recidivado, a pesar de resecciones amplias en más de diez ocasiones y de tratamiento radiante. Los objetivos de esta exposición son establecer la importancia que, ante todo tumor de partes blandas que se presente con un cuadro clínico de dolor y sobre todo en un enfermo de la tercera edad, se debe pensar en la posibilidad de que estemos ante a un Liposarcoma; precisar en nuestros cirujanos que hay que tener en cuenta esta posibilidad cuando estamos frente a un supuesto Lipoma y realizar una operación amplia y curativa en la primera intención. Para ello examinamos la Historia Clínica de nuestra enferma de 79 años de edad, exponiendo los datos vertidos en ella, o sea, cuadro clínico (dolor a tipo de “punzadas” a nivel de la cicatriz de las operaciones anteriores, donde vemos y palpamos un tumor duro y fijo a los tejidos adyacentes), los complementarios y la evolución que ha tenido en todo este tiempo que ya suman los doce años. Concluimos con las características a tener en cuenta de esta neoplasia (sumamente recidivante pero que no ha dado metástasis).Liposarcoma is a tumour that we have seen with little frequency in our medical practice. In this work done in the Surgical and Clinical Teaching Hospital Joaquin Albarrán, in the City of Havana 2004, we present you a case that we have been treated for over one decade ;it is about a 79 years old patient with a tumour of this variety that has grown again and again, in spite of big resections in more than ten occasions and with radiant treatment .the objective of this presentation is to make our surgeons see that they need to have in mind the possibility of this neoplasm when we are before a possible lipoma ,especially when it is presented by an old patient ,and with pain .For this ,we revised The Clinical History of our patient ,that gave the fundamental data part in it ;That is the clinical manifestation (pricking pain at the level of the scars of the former operation ,where we can see and palpate a little tumour ;hard and fixed to tissues around .The complementary tests and the evolution that the patient has had all this while time adds up to twelve years .We conclude with the characteristics to have in mind about this tumour (with high tendency to relapse, but that has not done metastasis in spite of the time of the evolution).
