Root Cause Analysis For Evaluating The Patient Falling With Fishbone Method

Abstract

In all standarts of Quality Management System and in approaches related to the extent and depth of corrective and preventive actions, root cause analysis is necessity of standarts. Health institutions uses root analysis to understand about cases why and how they occured, to deduce from events, to plan and implement processes and to avoid duplication of same or similar cases. Root analysiss' area of investigation is not subjects but cases and causes. The aim is not to find out whose negligence, but to make improvements in the system by observing all all the cases. The number of root cause analysis studies in the literature are little. Root cause analysis will improve problem-solving skills in medical malpractices that may develop or develop in health institutions. The purpose of this study is to guide the nurses by informing about fishbone method in root cause analysis and by giving an example about root cause applications.
