Serum levels of ICAM-I, VCAM, and Е-selectin in arterial hypertension with type 2 diabetes and obesity

Aim of this study was to determinate serum levels of ICAM-I, VCAM, and Е-selectin in patients whit arterial hypertension with or without type 2 diabetes mellitus and obesity. Material and Methods. We examined 41 hypertensive patients with type 2 diabetes, 18 obese hypertensive patients without diabetes, 10 normal weight hypertensive patients without diabetes, and 18 healthy control subjects. Serum levels of ICAM-I, VCAM, and Е-selectin were determined by immunoenzyme assay. The data were presented as means±SD. Statistical analysis was performed using the Mann-Whitney U-test, Pearson rank correlation coefficient, and multiple regression analysis. Results and Discussion. We found an increased serum level of ICAM-I in the hypertensive patients with type 2 diabetes, with and without obesity compared to the controls (253,11±51,09; 213,85±30,6; 177,66±27,59 and 147,48±6,06 ng/ml respectively, р<0,05). The serum level of VCAM was higher in hypertensive patients with type 2 diabetes, with and without obesity compared to the controls (736,53±100,87; 662,71±89,63; 518,3±70,32 and 437,33±37,69 ng/ml respectively, р<0,05). In thse patients the elevation of Е-selectin was compared to the controls (23,49±4,72; 19,02±3,53; 15,92±3,46 and 14,07±1,87 ng/ml respectively, р<0,05). We received a rise of ICAM-I, VCAM, and Е-selectin in hypertensive patients with diabetes compared to hypertensive patients with and without obesity (р<0,005). Also, the levels of ICAM-I, VCAM, and Е-selectin significantly correlated with HbA1c, serum insulin levels, HOMA, lipids, and body mass index (р<0,05). Conclusion. The revealed changes may reflect an endothelial dysfunction mostly pronounced in patients with arterial hypertension, type 2 diabetes mellitus, and obesity. Hyperglycemia, dyslipidemia, insulin resistance, and obesity appear to be significant contributing factors leading to the elevation of ICAM-I, VCAM, and Е-selectin levels.
