Y-shaped ileal prolapse through a patent omphalomesenteric duct: A case report

Background: Patent omphalomesenteric duct (OMD) is uncommon and has a prevalence of 0.0063–0.067% in the general population. The prolapse of small bowel through a patent OMD is even more uncommon. Immediate surgical treatment is required to prevent complications. Case presentation: A 17-day-old male presented with a protruding umbilical mass. The parents stated that after the umbilical cord fell off on day of life 6, a red mass appeared in the umbilicus and persisted despite applying antibiotic cream. The neonate had no vomiting or any signs of intestinal obstruction. Physical exam revealed a y-shaped ileal prolapse through a patent OMD. The patient underwent surgical reduction of the prolapse with resection of the affected ileal segment, and an end-to-end anastomosis. The operation was done through a sub-umbilical incision. The patient recovered well after the operation and was discharged home on postoperative day 5. Conclusion: Although rare, a patent OMD should be included in the differential diagnosis when evaluating umbilical abnormalities in children. Once diagnosed, all patent OMDs require surgical treatment.
