Endovascular repair of abdominal aortic aneurysm has been the first choice of treatment over surgical repair if feasible due to many advantages such as short hospital stay, less peri-procedural complications and early recovery. Good bilateral vascular access with adequate sized arteries that are free of significant disease is essential for a successful EVAR. Endovascular repair has been deferred in as much as 40% of patients due to difficult vascular access. We present a patient who underwent percutaneous endovascular repair of a juxta renal abdominal aortic aneurysm using a unique single side access EVAR device who had severely diseased iliac arteries.
