A seven years-old boy presented with headache and progressive drowsiness. MRI demonstrated obstruction at the foramen of Magendie and foramina of Luschka. Child underwent an endoscopic third ventriculostomy (ETV). The postoperative course was uneventful. MRI scan showed a marked decrease in the size of the ventricles, showing therefore that cerebrospinal fluid flow through the ventricular system and subarachnoid space was restored de Oliveira RS and Ballestero MFM contributed equally to this manuscript
