GALECTIN-3 AND MATRIX METALLOPROTEINASE-1 IN THE PATOGENESIS OF MYOCARDIAL DYSSYNCHRONY IN PATIENTS WITH CHRONIC HEART FAILURE AND TYPE 2 DIABETES

Summary. Diabetes mellitus (DM) type 2 is a result of structural and functional abnormalities that lead to diabetic cardiomyopathy, which is defined as a primary disease process. Elevated serum glucose potentiates the processes of apoptosis and activation of e. Excess levels of fibrosis in the myocardium affects the conducting system of the heart and is manifested by different types of myocardial dyssynchrony (DM), which impairs the pumping function of the heart and contributes to the progression of chronic heart failure (CHF). We surveyed 82 patients, mean age (70.75 + 9.24) years, with type 2 diabetes and ischemic CHF with manifestations of DM, we studied the content of fibrosis markers in blood serum - galectin-3 and MMP-1. There was a trend to an increase in the level of galectin-3 with a progression of clinical manifestations of CHF, which means an increase of fibrosis of the myocardium and irreversible changes in the extracellular matrix with an increase in functional class of heart failure, which worsens the condition of patients with type-2 diabetes. In the presence of myocardial dyssynchrony, quantitative markers of fibrosis were increased, in spite of the presence of diabetes type 2. In further studies, becomes relevant determining of correction parameters of myocardial fibrosis, in patients with manifestations of DM on the background of diabetes type 2, the analysis of indicators of myocardial fibrosis, with indicators of immune inflammation, hemodynamic parameters in patients with type 2 diabetes. The study of parameters of carbohydrate metabolism and correlation of myocardial fibrosis in patients without diabetes type 2 and activity of fibrosis markers in different genders.
