Background. Acute hemolysis is a tempestuous life threatening disease. Finding the right couse as soon as possible is very important for the beginning of the right therapy. Because of its quick development there is a frequent lack of time to find exact causes and to prevent its development. Methods and materials. A case of 62-year old female patient with ulcerative colitis, sideropenic anemia and febrile neutropenia is presented. She received a transfusion of 2 units of packed red blood cells. The following day she developed massive acute intravascular hemolysis with shock and multiple organ failure. She died within 24 hours after receiving the transfusion.Results. If the outset of hemolysis begins after blood transfusion, we have to exclude unwanted side effects of such terapy as the reason for hemolysis.
