Surgical treatment of chronic common carotid artery (CCA) occlusion with hemodynamic instability has not been well studied because of the rarity of CCA occlusion and its strategic dilemma due to the difficulty of using ipsilateral superficial temporal artery (STA) as a donor.Herein, the authors describe the first reported case of CCA occlusion treated by endarterectomy of the CCA and external carotid artery followed by STA–middle cerebral artery bypass. This new technical variant is safe and feasible and can be expected to have good patency if some special conditions can be met preoperatively and intraoperatively.
