Pleomorphic Adenomas primarily present in the major salivary glands and are more rarely observed within the minor salivary glands. Even less common is the presentation of a pleomorphic adenoma within the minor salivary glands of the sinonasal cavity, with the nasal septum being most commonly involved. The most common presenting symptoms of these patients include nasal obstruction followed by events of recurrent epistaxis. Here, we present an interesting case of endonasal resection under nasal endoscopic visualization paired with ipsilateral nasal septal cartilage and mucosal resection for a recurrent nasal septum pleomorphic adenoma with associated bony remodeling.
