A Comparative Study on Quality of Life between Ileal Conduit Urinary Diversion and Orthotopic Neobladder Reconstruction after Radical Cystectomy

Introduction: Ileal Conduit (IC) urinary diversion and Orthotopic Neo Bladder (ONB) reconstruction may have an effect on the Health-Related Quality of Life (HRQoL) of patients. Aim: To compare the quality of life between patients who underwent IC urinary diversion or ONB reconstruction following radical cystectomy in patients with Muscle-Invasive Bladder Cancer (MIBC). Materials and Methods: This was a prospective longitudinal study conducted between January 2015 and August 2017. Patients of either sex aged more than 18 years who underwent radical cystectomy followed by IC or ONB for invasive bladder cancer were included in this study. Functional Assessment of Cancer Therapy-Bladder (FACT-BL) questionnaire was used to assess the HRQoL. The data obtained were tabulated, analysed and presented using descriptive statistics. Results: A total of 32 patients were included. Overall, the mean age was 55.63 and 54.46 years in IC and ONB groups, respectively. The mean time lag between Transurethral Resection of Bladder Tumour (TURBT) and radical cystectomy was slightly higher in patients from IC group (66.11 vs. 50.23 days; p=0.314) than ONB group. Preoperatively the mean Functional Assessment of Cancer Therapy: General (FACT-G) score in patients from IC group and ONB group was 61.11 and 61.54, respectively. The bladder cancer sub scale scores were similar in both groups. The patients from IC group had significantly higher physical well-being score than the ONB group at both 6 and 12 months. All the differences have been found to be statistically significant except the score of emotional well-being at six months. Conclusion: Results from this study demonstrated that ONB had a better QOL than IC group at 12 months.
