A heart transplant recipient presented with right heart failure. He had a history of implantable cardioverter-defibrillator implantation and extraction before undergoing heart transplantation 4 years earlier. A large fistula was found between the left subclavian artery and vein. Following successful percutaneous fistula closure, symptoms and right ventricular function improved.
