Pseudoquilotórax en reactivación tuberculosa

Abstract

El pseudoquilotórax se define como un derrame pleural de aspecto turbio o lechoso debido a un alto contenido de lípidos y que no está producido por lesión del conducto torácico, al contrario del quilotórax, cuya incidencia es mucho mayor. Presentamos el caso de un hombre de 87 años que ingresa por infección respiratoria y para estudio de derrame pleural, detectándose en TC torácica derrame pleural bilateral de predominio derecho. Se le realiza una toracocentesis diagnóstico-terapéutica guiada por ecografía torácica, observándose un líquido pleural de aspecto lechoso, compatible con pseudoquilotórax. Resume: Pseudochylothorax is defined as a pleural effusion that is cloudy or milky due to a high lipid content and is not caused by a thoracic duct injury, unlike the chylothorax, the incidence of which is much greater. We present the case of an 87-year-old man who is admitted for respiratory infection and for the study of pleural effusion, with a bilateral pleural effusion of the right predominance as the thoracic CT scan detects. A diagnostictherapeutic thoracentesis guided by thoracic ultrasound is performed, with a pleural fluid of milky appearance, compatible with pseudochylothorax.
