Inferior vestibular neuritis mimicking central vertigo: Interest of VHIT and VEMPs in a case report

Abstract

Objective: To describe a case of inferior vestibular neuritis manifesting with a severe dizziness attack. To review available literature on the clinical features and characteristic presentation of this disease. Methods: Case report with literature review. Results: We report the case of a 54 years old female patient presenting with a severe vertigo without any other particular oto-neurological signs. The initial presentation simulated central vertigo, but the VHIT showed a left posterior canal deficit and the cVEMP was in favor of saccular involvement. All these elements would suggest an inferior vestibular neuritis. Conclusion: Inferior vestibular neuritis is a diagnosis to be evoked in front of an atypical vertigo and to be confirmed by the VHIT and the VEMPs.
