Retroperitoneal teratomas are rare tumors that are usually asymptomatic and often escape detection until they become very large. Here, we present the case of an asymptomatic 6-month-old boy referred for evaluation of abdominal distention who was found to have an 18 cm retroperitoneal teratoma. The mass was only loosely attached to surrounding structures and was completely excised, with pathology demonstrating a mature teratoma.
