Meta-analysis of alprostadil E1 on prevention of contrast-induced nephropathy
Abstract
Objective The incidence of Contrast-induced nephropathy(CIN) is increasing yearly,but the control effect on it hasn’t met the demand of people.There are reports that prostaglandin E1 has a certain effect on prevention of CIN,which lack of evidence-based medicine.We undertook a meta-analysis of the randomized controlled trails(RCTs) about prostaglandin E1(PGE1) preventing contrast-induced nephropathy(CIN) to asses the efficacy of PGEl on prevention of CIN.Methods Such databases as PubMed,Medline,Cochrane library,Chinese biomedical literature database,Chinese periodical full-text database and Wanfang database were searched from January of 2000 to June of 2014 to retrieve the RCTs.RevMan 5.1 software was used for Meta analysis.Results Fifteen RCTs involving 2 261 patients fit into the study.The meta-analysis of all RCTs showed that the incidence of CIN was significantly lower in the experimental group which take PGEl as the intervention measure than the control group which take placebo as the intervention measure[OR= 0.34,95%CK0.26,0.45),P<0.00001],we rejected the 5 RCTs without hydration and analysis again,die result showed that the incidence of CIN was also significantly lower in the PGEl + hydration group than the hydration group[OR = 0.36,95%CK0.25,0.51),P<0.00001].Conclusions PGE1 has a specific efficacy on prevention of CIN.PGEl combined with hydration is more effective than hydration alone on prevention of CIN.
