Anterior and posterior hip approach for treatment of extensive pigmented villonodular synovitis of the hip: A case report

Background: Pigmented villonodular synovitis (PVNS) is a rare synovial membrane disease characterized by proliferation and hyperplasia of the synovial membranes of the joints or tendons; it is a benign condition that can present as diffuse or focal, which leads to villonodular synovial hyperplasia and hemosiderin deposition in the synovial membranes of the affected joint. PVNS rarely affects the hip; typically, it affects the knee. Presentation of PVNS generally occurs between the third and fifth decades of life. Pain is the most common symptom of PVNS followed by occasional joint dysfunction. Case report: We present a case of PVNS of the hip that required anterior and posterior hip approaches for excision of extensive hip involvement. We demonstrate the characteristic presentation and magnetic resonance imaging PVNS features in the hip joint diagnosed using ultrasound-guided biopsy-proven cases. Conclusion: Because of the extensive hip involvement, this case represents a challenge in terms of surgical treatment. We describe the extent of this PVNS and present several surgical options for its treatment.
