The adenomatoid odontogenic tumor is a rare, non-aggressive, slow-growing and painless lesion that arises from the odontogenic epithelium. The maxillary region is the most affected one, predominating in the upper canine area. This study presents the case of a 46-year-old male patient of rural origin, who presented to the dental clinic due to a marked increase in volume on the right side of his face. Facial asymmetry was observed, caused by swelling of the right hemifacial region, which ranged from the right mandibular angle to the chin region of the same side, with a hard consistency. The skin of the area was normocolored. The patient was asymptomatic. The panoramic radiograph showed radiolucent areas that suggested bone lysis, not associated with a retained tooth. CT scan of the facial mass and incisional biopsy were performed, which allowed the diagnosis of an adenomatoid odontogenic tumor. Hemimandibulectomy with disarticulation was performed.
