The care of the aged becomes a critical priority in the ever-changing field of healthcare, requiring a specialized, and sensitive approach. This population is at the intersection of medical innovation and ethical care, as they are susceptible to the compounding effects of various drugs and complex health issues. In this regard, the Clinical Pharmacological Reconciliation, Review, and Feedback methodology is a trailblazing approach that seeks to improve and optimize care for the aged by applying the concepts of clinical pharmacology. This review explores few facets of this transformative approach.
