Primary mediastinal large B-cell lymphoma (PMBL) is an uncommon and aggressive subtype of non-Hodgkin large B-cell lymphoma that affects patients in the third and fourth decades of life. The objective of the present report is to describe the case of a male patient admitted to the emergency room with symptoms of dyspnea associated with signs of superior vena cava syndrome. The imaging exams revealed two masses in the mediastinum; the definitive diagnosis, established through an anatomopathological examination and immunohistochemistry, was of PMBL, and the patient was referred for chemotherapy.
