Abstract Hydrocephalus is rare in sarcoidosis, especially as the presenting symptom. Neurosarcoidosis as a cause of unexplained communicating hydrocephalus should be considered in cases of abnormal cerebrospinal fluid (CSF) and negative infectious and tumoral studies.
