A malignant mixed mullerian tumour (MMMT) is a rare tumour that primarily affects the uterus. MMMT of the ovary is very rare, contributing to 1% of ovarian carcinomas. It is an aggressive cancer of the ovary with characteristics of two types of cancers—carcinoma and sarcoma. A common presentation is a postmenopausal lady with a mean age of 65 years with complaints of abdominal mass, pelvic pain, and vaginal discharge. We report here a rare presentation of abnormal uterine bleeding in a peri-menopausal woman with an abdomino-pelvic mass, abdominal distension , post-coital bleed and abdominal pain, who underwent staging laparotomy with a preoperative diagnosis of malignant ovarian lesion and was found to have a malignant mixed Mullerian tumour of the ovary on histo-pathological examination. She is doing well at present after receiving six cycles of chemotherapy for Stage 2A disease. Diagnosis of malignant mixed mullerian ovarian tumours are difficult to confirm clinically because histopathological examination reports are required. This entity is rare and has poor prognosis. However, our patient is doing well to date.
