We present the case of a 53-year-old male from Veracruz, Mexico, admitted to the emergency department due to sudden left arm pain, paresis, cyanosis, and hypothermia. The extremity force was 2/5 on the Medical Research Council Scale (MRCS), and the vibratory and thermal sensitivity were abolished. Normal axillary pulse and absence of humeral, ulnar, and radial pulses in left arm. The contralateral upper limb had no apparent manifestations. A thromboendarterectomy was performed. The elected approach was at the humeral level and was explored proximal and distal to the surgical incision. A clot was extracted at the level of the bifurcation of the humeral artery.
