A 25-year-old primi, with a history of occipto-cervical fusion (OCF) performed at the age of 13 years, presented for an elective cesarean section. A combined spinal–epidural anesthetic (CSE) was administered, with a successful maternal and fetal outcome. The spinal component of the CSE was unsuccessful at providing surgical anesthesia.
