Results of the conservative treatment of patients with postnecrotic pancreatic cysts

Background. The high frequency of the postnecrotic pancreatic cysts is the result of restrained tactics for the treatment of patients with acute pancreatitis. In the literature there are conflicting data on the effectiveness of conservative treatment of postnecrotic pancreatic cysts. The purpose of the study is to evaluate the results of the conservative therapy in patients with postnecrotic pancreatic cysts. Materials and methods. The study included 22 patients with postnecrotic pancreatic cysts underwented conservative therapy. Indications for the conservative therapy were: the size of the postnecrotic pancreatic cysts is less than 5 cm, the patient has severe concomitant pathology in the stage of decompensation, the inability to perform minimal invasive interventions and the patient's refuse of surgical treatment. Results. The effectiveness of conservative therapy in patients with postnecrotic pancreatic cysts was 81.8% (n = 18), was characterized by relief of pain, lack of dyspeptic disorders, a decrease in the size of the postnecrotic pancreatic cysts or its complete obstruction. Complications after the use of conservative treatment developed in 2 (9.1%) patients: abscessing - 1 (4.5%), bleeding in the cyst cavity - 1 (4.5%). The recurrence in the form of an increase in the size of the cyst was observed in 5 (22.7%) patients, in 1 (4.5%) case the size of the cyst did not change in any way. Conclusions. The high efficiency of conservative therapy in patients with postnecrotic pancreatic cysts is determined by correct choice of indications.
