Estimation of the treatment efficacy in the wounded persons with the gun–shot polystructural damages of upper extremities
Abstract
Objective. To estimate the results and to reveal the rik factors for low efficacy of treatment in wounded persons with a gun-shot polystructural damages of upper extremities (UE). Маterials and methods. The UE function was estimated in 220 wounded persons, composing a general totality of the investigation, using two complementary procedures: DASH (Disability of the Arm, Shoulder and Hand Outcome Measure) and the AOOS systems in modification of І. М. Кurinnyi. Results, obtained in accordance to both approaches are consistent, what confirms their correlational analysis: r=0.883 – before the treatment, r=0.964 – after the treatment. Results. There was confirmed the efficacy of the surgical treatment tactics for the wounded persons with a gun-shot polystructural injuries of the UE. This tactics consisted of integration of several stages of reconstructive-restorational interventions, what was objectivized by determined indices of the extremity function growth. Conclusion. The dependence of the UE function restoration from the day of the wounding obtained and till the beginning of reconstructive-restorational interventions in institutions of Level IV was followed-up. This dependence was substantiated in accordance to index of the function growth and its enhanced efficacy was proved, when the terms noted have constituted lesser than 30 days. The risk degree for negative impact of the factors on the UE function restoration was estimated.
