Surgery for the treatment of severe obesity has a definite role onthe therapeutic armamentarium all over the world. Initiated 40years ago, bariatric surgery has already a long way thanks tohundred of surgeons, who had constantly searched for the besttechnique for the adequate control of severe obesity. Among theimportant breakthroughs in obesity surgery there is theadjustable gastric band. It is a sylastic band, inflatable andadjustable, which is placed on the top of the stomach in order tocreate a 15-20 cc pouch, with an outlet of 1.3cm. The adjustablegastric band has also a subcutaneous reservoir through whichadjustments can be made, according to the patient evolution.The main feature of the adjustable gastric band is the fact thatis minimal invasive, reversible, adjustable and placedlaparoscopically. Then greatly diminishing the surgical traumato the severe obese patient. Belachew and Favretti’s techniqueof laparoscopic application of the adjustable gastric band isdescribed and the evolution of the technique during this years,as we has been practiced since 1998. The perioperative care ofthe patient is also described, as well as the follow-up and shortand long term controls.
