This article provides a general overview of the pathway to become an intensivist in the United States of America (USA). It begins with a detailed description of the current structure of critical care training in the USA and the application process to fellowship training programs. There is then a presentation of the general structure of critical care units within the USA and how intensive care unit physicians fit into it. The article concludes with a discussion of how American critical care fits globally within international critical care societies.
