The cases of follicular lymphoma (FL) transformation have been reported, whereas FL transformation with skin lesions as the first sign has been rarely seen. In this article, we reported a 53-year-old female patient diagnosed with FL transformation into diffuse large B-cell lymphoma (DLBL) with multiple skin nodules of the breast as the first sign. The medical history of this patient was inquired and clinical diagnosis was confirmed by skin histopathology and immunohistochemistry. R2+ICE chemotherapy regimen (R2: rituximab + lenalidomide, I: ifosfamide, C: carboplatin, E: etoposide) was given. In addition, hepatopoietic stem cell transplantation was recommended. Nevertheless, the patient’s family discontinued further treatment and the patient died at 2 months after discharge.
