Following a review of the paper post-publication, the authors realized there were a few errors that were missed when the paper was revised before publication. Page 691, Abstract, Results section, line one, the text “Of 6311 patients” should read “Of 6113 patients”. Page 693, Results section, line one, the text “A total of 6311 eligible patients” should read “A total of 6113 eligible patients”.
